Results
If you wish to obtain the result of a test please telephone reception at the end of morning surgery. The doctor will return your call as soon as convenient. The result can only be given by the doctor to the patient concerned unless authorised by you. Receptionists cannot give or comment on results as they are not medically qualified.

Registration
If you are within the practice area and entitled to NHS care you will be given an afternoon appointment to see the doctor. Please attend reception with a passport or photo ID card, utility bill and your medical card if you have it. You will be registered and the doctor will record your basic health information (eg. past medical history, height, weight, blood pressure).

Overseas Visitors
Are not entitled to NHS care and will be treated as private patients unless from a country with whom there is a reciprocal agreement eg. EC or in an emergency.

If you are too ill to come to the surgery and need a visit please telephone if possible before 10.00 am.

Emergencies at Night and Weekends
Please telephone 01483 303200 and you will be automatically connected to the Thamesdoc switchboard. They will relay your request to the mobile doctor on call or arrange for you to be seen in an emergency centre. NHS Direct 0845 46 47

Sickness Certificates
A sickness certificate from a doctor is only required after 1 week. If less than this your employer will issue you with a self-certification form SC2.

Health Checks
We provide a 3-yearly health check on all patients over 40 years of age. Please make an appointment with a Practice Nurse via reception. A health check will be carried out and appropriate blood tests. The details will be available one week later.

Repeat Prescriptions
Please leave your request at reception by using the counterfoil from your last prescription. Your prescription will be ready for collection 24 hours later. Alternatively send a stamped addressed envelope with your request. We also operate a prescription delivery service with several local chemists. We do not accept repeat prescription requests by phone.

Patients Rights and Responsibilities

We will:
  • Ensure our patients have 24 hour access to medical care.
  • See any patient without an appointment on the morning of their choice
  • (Monday to Friday) in open surgery.
  • Involve you and listen to your opinions and views in all aspects of your medical care.
  • Arrange access to an interpreter given reasonable notice

What you can do for us:
  • Please let us know if you need to cancel an appointment, are running late or have missed an appointment.
  • Inform the practice staff of any change in your surname, address or telephone number.
  • Please treat all staff with courtesy and respect. We operate a zero tolerance policy toward physical and verbal aggression to any member of our staff.

Travel Immunisations
Travel immunisation by appointment with the Practice Nurse preferably at least 4 weeks prior to departure. Please bring details of areas to be visited. A charge (payable on administration) will be made for certain vaccinations that are not covered by the NHS. A list of these charges is held at reception.

Chaperone
You may be accompanied to a consultation by a friend, relative or chaperone.

District Nurses
The District Nurses provide home nursing for housebound patients eg. elderly, chronically sick or post-operative.

Practice Nurses
The Practice Nurses are happy to discuss any health problem and assist in the prevention of illness. They also carry out the routine annual medicals, asthma checks, blood pressure and diabetic clinics, carry out immunisations, blood tests and cervical smears, as well as general nursing duties eg. removal of sutures and ear syringing.

Health Visitors
The Health Visitor is attached to the surgery to assist mothers with children under the age of 6 years. She has a regular clinic in the Surgery. Please contact via reception.

Counselling
Counselling can be arranged within the PCT for minor psychological problems. Please see your doctor in the first instance.

Midwives
Our Midwife works as part of the surgery team - please telephone 303200. She carries out a weekly ante-natal clinic and will also visit post-natally at home.

Phlebotomist
The Phlebotomist is available twice a week for blood tests and blood pressure checks. She can also give some injections.

Confidentiality
Your information is stored on computer and we are registered under the Health Protection Act. We will only ever use or pass on information about you if others involved in your care have a genuine need for it. We will not disclose your information to third parties without your permission unless there are exceptional circumstances, such as when the health or safety of others is at risk or where the law requires information to be passed on. The new NHS computer system shares patient information between GPs and hospitals etc. If you are not willing for your information to be uploaded onto the spine please inform the doctor who will bar the transmission. We are fully compliant with the Freedom of Information Act 2000. By law you have the right to request access to your health records. Please contact us if you wish to apply for this access.

Complaints and suggestions for improvement
We are all keen to give a high standard of service reflecting your personal needs. If you have suggestions for improving the service please let us know, or leave a note at reception. If you have a complaint, either of a general nature or about the handling of a specific problem, we should like to hear about this also. We like to know where problems exist, both to put them right and for future improvement. We hope that most problems can be sorted out easily and quickly at the time they arise and with the person concerned. If your problem cannot be sorted out in this way and you wish to make a complaint, we would like you to let us know as soon as possible - ideally, within a matter of days or at most a few weeks. Written complaints should be addressed to the Pracitce Manager who will explain the complaints procedure to you and will make sure that your concerns are dealt with promptly. It will be a great help if you are as specific as possible about your complaint.

St Nicolas Surgery has a ramp and wheelchair-width doors throughout. There is also a WC for the disabled.
